Mixed Bacterial-Fungal Otitis Externa
Mixed bacterial-fungal otitis externa is an infectious and inflammatory condition affecting the skin of the external auditory canal, in which bacterial pathogens and pathogenic or opportunistic fungi are present simultaneously. The most characteristic symptoms include itching, pain, a feeling of pressure or blockage in the ear, tenderness when pressing on the tragus, swelling of the ear canal, redness, oozing, an unpleasant odor, whitish, gray, yellow, brown, or black discharge, crusting, and temporary hearing loss. The bacterial component is most commonly associated with Pseudomonas aeruginosa, Staphylococcus aureus, and other skin bacteria, while the fungal component is associated with Aspergillus, Candida, and, less commonly, other micromycetes. Infection may develop after water enters the ear, trauma caused by cotton swabs, scratching, use of in-ear headphones or hearing aids, use of topical antibiotics, or disruption of the skin's acidity and protective lipid layer. A mixed infection is especially likely after incomplete or incorrectly selected treatment of bacterial otitis, when an antibiotic suppresses part of the bacterial flora but creates conditions favorable for fungal colonization. The primary target tissues are the epidermis of the ear canal, hair follicles, sebaceous glands, and ceruminous glands; in severe cases, inflammation may spread to the auricle, tympanic membrane, soft tissues, and bony structures at the base of the skull. In children, swelling can obstruct the narrow ear canal more quickly; in older patients, particularly those with diabetes mellitus, the risk of invasive disease is increased; in adults, the condition is more often associated with water exposure, ear trauma, and topical self-treatment. Fungal and mixed necrotizing otitis externa are rare but occur predominantly in immunocompromised patients and may be accompanied by mixed microbial flora.
Initial Self-Care Measures for Mixed Bacterial-Fungal Otitis Externa
Until medical examination, water must be kept completely out of the affected ear, and swimming, baths, saunas, in-ear headphones, earplugs, and use of a hearing aid on the affected side should be avoided if temporarily removing the hearing aid is safe. During a shower, the outer opening of the ear canal may be protected with a dry cotton ball lightly coated with petroleum jelly on the outside, without pushing it into the canal; the protection should be removed immediately after the shower. The most dangerous complication is necrotizing otitis externa with osteomyelitis of the temporal bone and involvement of the cranial nerves. Recurrences of otomycosis are not uncommon, especially after ear surgery and in patients with immune disorders.

For internal use in recurrent or prolonged disease, the preferred powdered extract is Coptis chinensis. Its alkaloid complex is considered a systemic antimicrobial, antifungal, and anti-inflammatory component, particularly when otitis externa is accompanied by other manifestations of dysbiosis, candidiasis, or metabolic disorders. Coptis does not replace local debridement or pathogen-directed otic medications. It is contraindicated in individual hypersensitivity, marked gastric hypoacidity, and chronic hepatitis with cholestasis. Possible adverse effects include nausea, a bitter taste in the mouth, reduced appetite, abdominal discomfort, and headache. The possibility of enhanced effects of glucose-lowering, antihypertensive, anticoagulant, and antiarrhythmic drugs should be taken into account, as should interactions with medications metabolized through CYP3A4 and P-glycoprotein.
Nigella sativa in powdered-extract form is used orally as an additional anti-inflammatory, immunomodulatory, and antimicrobial component. It may be particularly appropriate when infection is accompanied by eczematous, atopic, or allergic inflammation of the skin of the ear canal and a tendency to recur. Additional oil-based otic forms of Nigella are not used simultaneously with ABP-153 because this would duplicate topical therapy. Allergic reactions, dyspepsia, loose stools, and reductions in blood pressure or blood glucose may occur. Caution is required in hypotension, hypoglycemia, and during use of anticoagulants, antiplatelet drugs, antihypertensive medications, and glucose-lowering agents.
Curcuma longa in powdered-extract form is used orally as an anti-inflammatory and antioxidant component that supports restoration of damaged tissues. It does not replace antimicrobial or antifungal treatment and has a lower priority than Coptis. Turmeric is contraindicated in biliary obstruction, exacerbation of gallstone disease, active ulcerative lesions of the gastrointestinal tract, and clinically significant bleeding. Heartburn, nausea, abdominal pain, and diarrhea may occur. Combination with anticoagulants or antiplatelet agents may increase the risk of bleeding.
Scutellaria baicalensis in powdered-extract form may be included when there is a pronounced inflammatory-allergic component, persistent itching, irritability, or sleep disturbance caused by pain. Its role remains supplementary. Drowsiness, dizziness, weakness, and decreased blood pressure are possible. Scutellaria may enhance the effects of hypnotic, sedative, anxiolytic, and anticonvulsant drugs. Caution is required in liver disease, hypotension, and work requiring a high level of concentration.
The Five Root Compound anti-inflammatory formula in powdered form is taken orally when inflammatory pain and swelling of the ear canal are pronounced. It does not mechanically remove accumulated fungal debris and does not replace antibacterial or antifungal therapy. Its clinical purpose is to reduce the intensity of inflammation, pain, and tissue edema without the typical risks associated with systemic NSAIDs, such as erosive and ulcerative gastric injury, gastrointestinal bleeding, fluid retention, and impaired renal perfusion. However, individual intolerance to plant ingredients, nausea, abdominal pain, or diarrhea may also occur. The formula contains Harrisonia perforata, Capparis micracantha, Clerodendrum petasites, Ficus racemosa, and Tiliacora triandra.
The Antipyretic Compound is not part of the basic treatment of uncomplicated otitis externa. It may be considered only in confirmed fever after a physician has excluded otitis media, perichondritis, cellulitis, a necrotizing process, and spread of the infection.
