Aspergillus Otomycosis - Black or Gray Discharge, Severe Itching, and a Plugged-Ear Sensation
Aspergillus otomycosis is a fungal infection of the external auditory canal caused by molds of the genus Aspergillus. Its most recognizable symptoms are severe itching, a sensation of a plug or foreign body in the ear, ear fullness, and reduced hearing. The discharge may be gray, brownish, or almost black. Small black specks are particularly characteristic of Aspergillus niger, although the color of the discharge alone cannot confirm the causative organism.
Other possible symptoms include scaling of the skin, an unpleasant odor, moderate pain, tinnitus, and tenderness when pressure is applied to the tragus. The condition develops when fungal spores reach moist or damaged skin in the ear canal.
The risk is increased by a hot and humid climate, swimming, the use of cotton swabs, scratching the ear canal, earphones and hearing aids, prolonged use of topical antibiotics or glucocorticosteroids, chronic otitis externa, eczema, diabetes mellitus, and immunodeficiency.
The main site of infection is the skin of the external auditory canal. The tympanic membrane is affected much less frequently. In children, a foreign body in the ear must be ruled out. In older adults and patients with diabetes, it is particularly important not to miss the spread of infection.
With timely cleaning and appropriate treatment, an uncomplicated episode usually resolves within several weeks. However, recurrence is possible unless persistent moisture and repeated trauma to the ear canal are eliminated.

The condition may be suspected when deep itching in the ear is accompanied by increasing fullness, impaired hearing, and dark or gray loose discharge, especially after swimming, visiting a pool, cleaning the ear independently, or using antibacterial ear drops.
Visible black specks, grayish-white filaments, and loose masses may represent Aspergillus spores and mycelium. However, you must not attempt to inspect the ear canal with objects or remove its contents yourself.

Red Flags
Severe or rapidly worsening pain, particularly pain that intensifies at night, spreads to the temple, jaw, or neck, marked swelling of the ear canal, high fever, and deterioration in general condition require urgent assessment by an otorhinolaryngologist.
Bloody discharge, sudden significant hearing loss, dizziness, vomiting, impaired balance, weakness of the facial muscles, facial numbness, or severe headache may indicate damage to the tympanic membrane, involvement of the middle or inner ear or the facial nerve, or spread of the infection.
Patients with diabetes mellitus, immunodeficiency, cancer, or neutropenia, as well as those who have undergone transplantation or are receiving immunosuppressive therapy, must not delay seeking medical care if they develop ear pain and discharge. In such patients, invasive infection involving the tissues and the base of the skull must be excluded.
A young child, a pregnant woman, or an older patient whose condition is deteriorating rapidly should be examined by a physician promptly. Altered consciousness, pronounced facial muscle weakness, or other neurological symptoms require emergency medical care.
Initial Self-Care Measures for Aspergillus Otomycosis
Until you are examined by a physician, the ear should be kept dry. Avoid swimming, diving, saunas, and allowing water to enter the ear while showering.
To protect the ear from water, a small cotton ball lightly coated with petroleum jelly may be placed at the entrance to the auricle. It must not be pushed into the ear canal and should be removed immediately after showering.
Only the outer part of the auricle may be gently blotted with a clean, dry tissue. Do not use cotton swabs, tweezers, hairpins, ear candles, hydrogen peroxide, alcohol, vinegar, boric acid, essential oils, herbal decoctions, warming compresses, or self-administered ear irrigation.

No special diet or increased fluid intake is required. A normal diet and your usual amount of water are acceptable, taking into account any restrictions related to heart or kidney disease and blood glucose levels.
Monitor the intensity of pain, body temperature, the amount of discharge, hearing, dizziness, and swelling. Patients with diabetes should monitor their blood glucose.

Stages and Possible Progression of Aspergillus Otomycosis
The disease does not have a formally defined stage-by-stage course. It usually begins with itching, dryness, and a sensation of irritation. As mycelium, spores, desquamated epithelium, and earwax accumulate, ear fullness, a plugged-ear sensation, discharge, and hearing loss develop.
If the skin is damaged, a bacterial infection may develop in addition to the fungal process, causing pain, swelling, and pus. Jindu Ear Drops form the basis of topical integrative therapy. They are used after fungal masses have been professionally removed from the ear canal and the integrity of the tympanic membrane has been confirmed.
Chlorhexidine acetate provides direct antiseptic and antifungal activity, while costus, vitex, dendrobium, and borneol complement its anti-inflammatory, antipruritic, anti-edematous, and reparative effects.
The product must not be used in cases of tympanic membrane perforation, the presence of a drainage tube, weeping dermatitis, allergy to chlorhexidine, camphor, or costus, or in children under six years of age. Temporary burning, irritation, redness, and allergic contact reactions may occur.
The ABP-153 oil infusion is suitable for superficial and cavity-localized Aspergillus otomycosis. Clove and the proprietary components of the formula provide antifungal and antiseptic activity. Turmeric, andrographis, cassumunar ginger, and Houttuynia cordata reduce the inflammatory response, while menthol and borneol help relieve itching, pain, and swelling.
ABP-153 should be used only after the external auditory canal has been cleaned and only when the tympanic membrane is intact. It must not be mixed simultaneously in the ear canal with Jindu Ear Drops, clotrimazole, or another topical agent. The products should be used sequentially according to the condition of the skin and the therapeutic objective.
ABP-153D is not required for superficial otomycosis because enhanced penetration into deeper tissues is not clinically justified. Contraindications include damage to the tympanic membrane and individual hypersensitivity. Local burning, redness, and irritation may occur.
Topical herbal formulations place minimal systemic burden on the liver, kidneys, gastrointestinal tract, cardiovascular system, and nervous system. They do not produce the numerous hepatic enzyme-mediated interactions associated with systemic azoles and do not cause negative inotropic effects, systemic phototoxicity, or pronounced hepatotoxicity.
However, topical therapy does not replace professional removal of fungal masses. In cases of persistent, laboratory-confirmed Aspergillus infection, the need for clotrimazole, voriconazole, or another antifungal agent is determined by an otorhinolaryngologist based on otoscopy, microscopy, culture, and assessment of fungal susceptibility.
Nigella sativa is used as a 10% oral extract rather than as an otic preparation. Thymoquinone, thymohydroquinone, and thionins from black seed may inhibit mycelial growth and spore formation in several Aspergillus species while also reducing the inflammatory response.

Scutellaria baicalensis is used as a 10% oral extract. Baicalein and baicalin affect Aspergillus growth, adhesion, and biofilm formation and may also reduce the local production of inflammatory mediators.
Its clinical role is to support the management of pronounced itching, swelling, and inflammation of the ear canal. Possible adverse effects include nausea, reduced blood pressure, drowsiness, tremor, and muscle weakness. Caution is required in liver disease and with concurrent use of sedatives, antihypertensive agents, anticoagulants, and antiplatelet drugs.
Curcuma longa is used only as a 10% oral extract. Curcuminoids reduce inflammation, oxidative damage, and swelling, support epithelial recovery after cleaning of the ear canal, and demonstrate preclinical activity against Aspergillus.
The extract must not be used in cases of biliary tract obstruction, acute pancreatitis, or exacerbation of gallstone disease. Possible adverse effects include dyspepsia, heartburn, nausea, and loose stools. When used with anticoagulants or antiplatelet agents, the possibility of increased bleeding must be considered.
Coptis chinensis is a reserve 10% oral extract. Berberine affects membrane and metabolic mechanisms in fungal cells, but because Coptis has numerous drug interactions, it should be used only after a complete review of the patient’s pharmacotherapy.
Possible adverse effects include nausea, abdominal pain, constipation or diarrhea, reduced appetite, lower blood pressure, and reduced blood glucose. Berberine alters the activity of CYP3A4, CYP2D6, CYP2C9, and P-glycoprotein and may increase the concentrations of cyclosporine, tacrolimus, and a number of other medicines. It must not be used during pregnancy, breastfeeding, or infancy.
Five Root Compound anti-inflammatory mixture, Antipyretic Compound, and Aromatic Cooling Oil Sukaya are not included in the basic treatment regimen for localized Aspergillus otomycosis.
Fever, severe headache, general intoxication, and widespread inflammatory pain are not typical of an isolated external fungal process. These symptoms require the exclusion of a mixed bacterial infection, middle ear involvement, mastoiditis, or spread of inflammation beyond the ear canal.

Oral itraconazole is also not routinely used for localized Aspergillus otomycosis of the external auditory canal. It may be considered when the infection has spread, deeper tissues are involved, immunodeficiency is present, or adequate local control cannot be achieved.
Itraconazole may cause nausea, vomiting, abdominal pain, diarrhea, headache, dizziness, peripheral neuropathy, edema, and arterial hypertension. The most serious adverse effects include drug-induced hepatitis, cholestasis, liver failure, and a negative inotropic effect that may trigger or worsen heart failure.
Hypokalemia, QT interval prolongation, and dangerous ventricular arrhythmias may occur. The medicine is contraindicated during pregnancy, in severe liver disease, and in heart failure.
Inhibition of CYP3A4 creates a risk of critically increased concentrations of certain antiarrhythmic agents, statins, immunosuppressants, anticoagulants, benzodiazepines, and other medicines. Concomitant use without a complete review of the patient’s pharmacotherapy is dangerous.
