Toxicity and Biosafety of Dioscorea hispida

Toxicity studies of the rhizomes of Dioscorea hispida confirm the presence of pronounced dose-dependent toxic action when used in high concentrations. In raw form, the tubers of the plant contain dioscin and other steroid saponins, as well as cyanogenic glycosides (especially dioscorine), which have neurotoxic properties.

In experimental studies on mice, it was established that oral administration of Dioscorea hispida extract at a dose of 1000 mg/kg body weight caused signs of toxicity (diarrhea, hypoactivity, lethargy), but did not lead to death. The LD₅₀ upon a single administration of the alcoholic extract was about 2500 mg/kg for mice, which classifies this plant as subtoxic.

With prolonged use of the extract (for 28 days) at doses up to 200 mg/kg in rats, no significant deviations in clinical and biochemical blood parameters and in the function of the liver and kidneys were detected, which allows us to speak of an acceptable safety profile for therapeutic use after thermal or chemical detoxification of the raw material.

It is important to note that consumption of the raw tuber as food or as an infusion without prior neutralization of toxins can cause severe poisoning with symptoms of CNS depression, convulsions, and in rare cases — death, especially in children.

A decrease in spontaneous motor activity, suppression of conditioned reflexes, and potentiation of effects mediated by γ-aminobutyric acid (GABA) have been noted, indicating a possible effect on GABAergic neurotransmission.

At the level of the gastrointestinal tract, preparations of Dioscorea hispida have local and systemic effects. An antispasmodic effect is noted, realized probably through blockade of calcium channels of smooth muscles and mediated influence on parasympathetic regulatory mechanisms. A choleretic effect and participation in the stimulation of small intestinal motility have been confirmed. Due to the high content of saponins, plant components exhibit the ability to stimulate mucus secretion and protect mucous membranes from irritating factors.

The systemic anti-inflammatory effect of Dioscorea hispida extracts is associated with suppression of the synthesis of pro-inflammatory mediators, such as prostaglandins, through inhibition of cyclooxygenase-2 (COX-2) activity, as well as suppression of interleukin production (IL-1β, IL-6). An antioxidant effect has also been noted, expressed in the ability to bind reactive oxygen species and prevent lipid peroxidation, which contributes to the stabilization of cell membranes.

Pharmacokinetics — Dioscorea hispida

The pharmacokinetic characteristics of Dioscorea hispida depend on the form of administration, the composition of the raw material, and the degree of pre-treatment, taking into account the presence in the plant of both water-soluble and fat-soluble biologically active compounds. With oral administration of preparations from processed rhizomes, slow absorption of active substances in the gastrointestinal tract is observed, due to partial inactivation of saponins and glycosides under the action of gastric acid and enzymes. However, preliminary thermal and enzymatic treatment (including drying, boiling, alcohol extraction) significantly reduces toxicity and increases the bioavailability of active components.

The absorption of saponins and diosgenin in the intestine is limited and may depend on the state of the microflora, since bacterial transformation is a key stage of their activation. Scientific publications confirm the presence of synergistic interaction between extracts of Dioscorea hispida and plant components with similar or complementary pharmacological activity. Dioscorea hispida is traditionally used in the folk medicine of Southeast Asia, including Indonesia, Malaysia, the Philippines, Thailand, Vietnam, and the southern regions of China. The plant is most actively used on the islands of Java, Sumatra, and Kalimantan, where it is part of the ethnobotanical practices of the Malay, Sundanese, Javanese, and Batak ethnic groups. In traditional Indonesian and Philippine medicine, the plant was used in the form of boiled and dried tubers, which were then ground into a paste or powder. These forms were used for wraps, poultices, and ointments. In the culture of the Dayaks of Borneo, Dioscorea hispida tubers were also used externally in the form of warm cakes applied to the body in special purification ceremonies aimed at expelling the "spirit of disease." In some tribal cultures of Malaysia and the Philippines, the plant was considered a plant of "dual action": potentially dangerous with inept use and extremely valuable after ritual purification.

In the Thai tradition, Dioscorea hispida is known as "มันทรงกระเทียม" and is used mainly in the northern provinces, such as Chiang Mai, Lampang, and Phrae.
