Dosage of Pharmaceutical Forms — Derris scandens

Powder — Derris scandens

Indications (Powder): Knee osteoarthritis, lumbar pain syndrome, tendinitis, myofascial pain syndrome, chronic arthritis, inflammation of the ligamentous apparatus, degenerative spinal diseases.

Standard Dosage (Powder): 1.5 grams of powder 2 times a day, with warm water, for 10–14 days. Take 30 minutes before meals.

Enhanced Dosage (Powder): 2 grams 3 times a day, for severe forms of osteoarthritis, active phase of spondyloarthrosis, exacerbation of chronic arthritis, pain associated with progressive tendinitis or facet syndrome.

Maximum Dosage (Powder): No more than 9 grams per day (3 grams 3 times a day), used in conditions of pronounced pain syndrome with no response to the standard regimen. Course duration — no more than 7 days, with mandatory monitoring of liver function.

Preventive Dosage (Powder): 0.5–1 gram once a day, in the morning, on an empty stomach, for 20 days, for chronic osteoarthritis, aging of connective tissue, a sedentary lifestyle, and during long-term joint immobilization. Courses — 2–3 times a year.

Pediatric Dosage (Powder): From 12 years, with a body weight of at least 40 kg — 0.5 grams 2 times a day, for 5–7 days. No safety data for under 12 years have been registered.

Contraindications (Powder): Individual intolerance, acute inflammatory diseases of the gastrointestinal tract, gastric and duodenal ulcers in the acute phase, pronounced hepatic insufficiency. No scientific contraindications have been registered during pregnancy, lactation, and in children under 12 years.

Side Effects (Powder): When the dosage is exceeded, dry mouth, dyspepsia, dizziness, skin rash, abdominal pain, and transient tachycardia may develop. All effects resolve after discontinuation of the drug.

Adjustment for Patient Body Weight (Powder): For body weight less than 60 kg — reduce the dose by 20%. For body weight more than 90 kg — increase the dose by 25% while maintaining the daily limit of 9 grams.

Preparation method (Powder): To prepare 100 grams of powder, the following are required: dry mature stems and bark of Derris scandens — 100 grams. The raw material is washed, dried at a temperature of 50–60 degrees Celsius for 48 hours to a residual moisture content of no more than 10%. Then ground in a micromill to a fraction of less than 250 microns. The resulting powder is sieved through sieve No. 200 and packaged in airtight containers.

Extract — Derris scandens

Maximum Dosage (Extract): The daily dose should not exceed 3000 milligrams. Such a dosage is permissible short-term (no more than 5 days) in the absence of response to standard therapy, only under the control of liver function parameters.

Preventive Dosage (Extract): 250 milligrams of extract once a day for 20 days, during periods of seasonal exacerbation of joint diseases, with physical inactivity, after spinal surgeries. Courses are recommended to be repeated 2 times a year.

Pediatric Dosage (Extract): From 12 years and with a body weight of at least 40 kilograms — 250 milligrams once a day for 5 days. No safety data for under 12 years have been registered.

Contraindications (Extract): Hypersensitivity to components, gastric ulcer disease, acute pancreatitis, pregnancy and lactation — contraindications do not have sufficient scientific verification; use is possible only after assessing the risk and benefit. In children under 12 years, the contraindication remains.

Side Effects (Extract): The following have been observed: skin itching, heartburn, dizziness, flatulence, short-term increase in liver enzyme activity, a feeling of heaviness in the epigastric region in case of overdose. All effects disappear after discontinuation of the drug.

Adjustment for Patient Body Weight (Extract): For body weight up to 60 kilograms, it is recommended to reduce the dose by 25%. For body weight more than 90 kilograms, an increase in dose to 1200 milligrams 2 times a day is possible while maintaining the daily limit.

Preparation method (Extract): To prepare 100 grams of dry extract: ground dried stems of Derris scandens — 500 grams, 50% ethanol — 2000 milliliters. Infuse at a temperature of 70 degrees Celsius for 4 hours. Filter, concentrate at 45 degrees to a volume of 200 milliliters, dry in a drying chamber at 40–45 degrees to obtain a dry residue. Package in light-proof containers.

Storage Conditions and Shelf Life (Extract): Store in a dry, light-protected place, at a temperature of 15 to 25 degrees Celsius, in airtight containers. Shelf life — 18 months. After opening — use within 45 days, provided it is stored in a tightly closed container.


Alcohol-Based Tincture — Derris scandens

Indications (Tincture): Osteoarthritis with an inflammatory component, chronic back pain, myositis, ligament inflammation, reactive synovitis, lumbalgia, arthralgia in infectious joint lesions.

Standard Dosage (Tincture): 15 drops of tincture (approximately 0.75 ml) 2 times a day, diluted in 50 ml of warm water, 30 minutes before meals. Course — 10 days.

Enhanced Dosage (Tincture): 20 drops 3 times a day for pronounced joint pain, severe forms of spondyloarthrosis, as well as in the post-traumatic period with ligament sprains and joint capsule inflammation.

Maximum Dosage (Tincture): No more than 25 drops (1.25 ml) 3 times a day. Used only in conditions of short-term exacerbation (up to 5 days). Requires monitoring of liver function and general well-being.

Preventive Dosage (Tincture): 10 drops once a day, in the morning, on an empty stomach, for 14 days. Indicated for patients with chronic arthritis, a tendency to reactive joint inflammation, especially in spring and autumn. Courses — no more than 3 times a year.

Pediatric Dosage (Tincture): This form is contraindicated for children under 18 years due to ethanol content and the lack of safety data in pediatrics.

Contraindications (Tincture): Alcohol dependence, acute inflammatory diseases of the gastrointestinal tract, liver cirrhosis, hepatitis, mental disorders, pregnancy, lactation. Contraindicated in childhood. A complete contraindication in mild forms of hepatic insufficiency has not been scientifically registered — use is possible with caution.

Oil Infusion — Derris scandens

Indications (Oil Infusion): Local inflammations of joints and tendons, myofascial pain syndromes, reactive periarthritis, neurogenic pain, ligament inflammation, tendinitis, intercostal neuralgia, osteochondrosis with local edema.

Standard Dosage (Oil Infusion): Externally, apply 3–5 ml of infusion to the affected area 2 times a day with light massage movements. Course — 10–14 days.

Maximum Dosage (Oil Infusion): No more than 10 ml 3 times a day on an area of no more than 20×20 cm. Short-term use (up to 5 days) is permissible for acute pain syndrome. Do not apply to damaged skin.

Preventive Dosage (Oil Infusion): 2–3 ml once a day for 10 days for recurrent pain after physical exertion, chronic myalgia, a sedentary lifestyle, and during the rehabilitation period after injuries. Courses — 3–4 times a year.

Pediatric Dosage (Oil Infusion): From 7 years, with a body weight of at least 25 kilograms: 1–2 ml once a day, only on intact skin areas, for pain syndrome after physical exertion or mild arthralgia. Under 7 years — not recommended.

Contraindications (Oil Infusion): Allergy to components, dermatitis, eczema in the application area, pustular skin lesions, open wounds. During pregnancy and lactation, external use is possible, in the absence of irritation and only as indicated. Contraindicated in children under 7 years.

Side Effects (Oil Infusion): The following have been registered: skin hyperemia, itching, local burning, urticaria. In case of overdose, skin irritation, peeling, and contact dermatitis are possible. No systemic effects have been identified.

Add 0.1 grams of natural vitamin E as an antioxidant.

Dry Powder Extract — Derris scandens

Maximum Dosage (Dry Powder Extract): The daily dose should not exceed 3000 milligrams (1000 milligrams 3 times a day). Use no more than 7 days. Laboratory monitoring of liver and kidney function is mandatory when prescribing this regimen.

Preventive Dosage (Dry Powder Extract): 200 milligrams once a day in the morning on an empty stomach for 20 days. Used for chronic osteoarthritis in remission, degenerative spinal changes, physical inactivity, and during the period of physiological aging of articular cartilage. Course repetition — every 6 months.

Pediatric Dosage (Dry Powder Extract): Permissible from 12 years with a body weight of at least 40 kilograms: 200 milligrams once a day for 5–7 days. Under 12 years, safety has not been confirmed.

Contraindications (Dry Powder Extract): Individual intolerance, exacerbation of ulcerative disease, severe hepatic insufficiency, acute inflammatory diseases of the gastrointestinal tract. There are no scientifically confirmed contraindications during pregnancy, lactation, and in children under 12 years, however, use is not recommended without specialist consultation.

Side Effects (Dry Powder Extract): Rarely: dyspepsia, headache, skin rash, drowsiness, increased ALT and AST activity. When doses are exceeded, toxic-allergic reactions, a feeling of heat, and tachycardia are possible.

Adjustment for Patient Body Weight (Dry Powder Extract): For patients with body weight less than 60 kilograms — reduction of dosage by 25%. For weight over 90 kilograms, an increase in dosage by 20% is possible while observing the maximum daily threshold.

Preparation method (Dry Powder Extract): To prepare 100 grams of dry extract: dried and ground stems of Derris scandens — 500 grams, purified water — 3000 milliliters. Standard Dosage (Liquid Extract): 5 milliliters (approximately 1 teaspoon) 2 times a day, dissolved in 50 milliliters of warm water, after meals. Duration of intake — from 10 to 14 days.

Enhanced Dosage (Liquid Extract): 7.5 milliliters 3 times a day in the presence of pronounced pain syndrome, enhanced inflammatory component, or ineffectiveness of the standard regimen. Used in cases of reactive arthritis, severe facet inflammation, chronic joint pain due to cartilage degeneration.

Maximum Dosage (Liquid Extract): No more than 10 milliliters 3 times a day (30 ml/day) short-term, no longer than 5 days. Monitoring of liver function is mandatory, especially with concomitant gastrointestinal diseases.

Preventive Dosage (Liquid Extract): 3 milliliters once a day, dissolved in warm water, in the morning on an empty stomach. Used for a tendency to inflammatory joint changes, physical inactivity, in old age, and after prolonged loads. Courses — 14 days, 2–3 times a year.

Pediatric Dosage (Liquid Extract): From 12 years, with a body weight of at least 40 kilograms — 2.5 milliliters once a day, for 5 days. Under 12 years, use is not recommended due to the lack of an evidence base.

Contraindications (Liquid Extract): Hypersensitivity to components, severe liver diseases, active ulcerative disease, pregnancy and lactation — contraindications have not been scientifically confirmed, however, it is recommended to avoid use without medical supervision. In children under 12 years — contraindicated.

Side Effects (Liquid Extract): Scientifically recorded: dyspeptic phenomena, dizziness, short-term tachycardia, facial flushing, skin itching. All effects resolve after discontinuation of the drug.

Topical Patch — Derris scandens

Indications (Topical Patch): Localized pain in osteoarthritis, chronic pain syndrome in the lumbar region, inflammations of periarticular soft tissues, myositis, tendinopathies, periarthritis, facet syndrome of the spine.

Standard Dosage (Topical Patch): One patch of 10×14 cm is applied to the painful area for 6–8 hours once a day. Course — 7 days.

Enhanced Dosage (Topical Patch): One patch 2 times a day (morning and evening), each for 6 hours, for pronounced local inflammation, acute pain syndrome, impaired mobility, including lumbalgia and radicular syndromes.

Maximum Dosage (Topical Patch): No more than three patches within 24 hours (applied to different areas, do not overlap). Permissible for resistant pain, no more than 5 days in a row.

Preventive Dosage (Topical Patch): One patch every other day at night, course — 5 applications. Used for chronic osteoarthritis, static loads, occupational risks in drivers, operators, and people working standing for long periods. Prevention is carried out 2–3 times a year.

Pediatric Dosage (Topical Patch): From 14 years: 1/2 of a standard patch (cut) for 4 hours once a day. Under 14 years, contraindicated due to increased skin permeability and the lack of an evidence base.

Contraindications (Topical Patch): Pustular rashes in the application area, dermatitis, psoriasis, eczema, increased skin sensitivity, allergy to patch components. Pregnancy, lactation — only short-term external use on small areas is permissible. Contraindicated for children under 14 years.

Side Effects (Topical Patch): Scientifically registered: contact dermatitis, itching, redness, peeling, skin hyperesthesia. In rare cases — irritation and local pain.

Adjustment for Patient Body Weight (Topical Patch): Adjustment is not required, as the form is intended for local use. For body weight less than 50 kilograms — limit the application area to one area up to 10×10 cm.

Preparation method (Topical Patch): For 100 patches: thick extract of Derris scandens — 200 grams, crystalline menthol — 5 grams, camphor — 3 grams, base (mixture of polyvinyl alcohol and gelatin) — 300 grams. Dissolve the extract in the base, add menthol and camphor, mix until homogeneous, apply evenly in a 2 mm layer on a fabric backing and dry at a temperature of 40 degrees for 12 hours. Cut into strips of 10×14 cm, package in airtight individual bags.

Storage Conditions and Shelf Life (Topical Patch): Store at a temperature of 5 to 25 degrees Celsius, in a dry, light-protected place. Avoid exposure to direct sunlight and heat. Shelf life — 24 months. After opening the package, use within 12 hours.


Cosmetic Product (Lotion/Milk) — Derris scandens

Indications (Cosmetic Product): Aging skin with signs of inflammation, dryness and peeling, tendency to local edema, rosacea, skin hypersensitivity, dermatomyalgia, inflammatory processes in the area of joints and muscles in mild form, skin sensitivity after exertion.

Standard Dosage (Cosmetic Product): Apply 1–2 ml of lotion or milk to cleansed skin 1–2 times a day, evenly distributing in a thin layer over the affected or sensitive area. Course — 14 days.

Enhanced Dosage (Cosmetic Product): Use up to 3 times a day, 2–3 ml each, for pronounced skin sensitivity, inflammatory reactions after physical exertion, irritation in flexion areas, in patients with pronounced signs of skin dehydration or local reactivity.

Maximum Dosage (Cosmetic Product): Up to 4 ml 3 times a day on an area up to 100 cm². Continuous use for a maximum of 7 days is permissible, then a break of at least 5 days is recommended. Do not apply to damaged skin or mucous membranes.

Preventive Dosage (Cosmetic Product): 1 ml once a day for 10 days for seasonal dry skin, a tendency to redness, reactivity after heat or cold, age-related skin changes, as part of preventive care in persons over 40 years. Repeat the course once every 2 months.

Pediatric Dosage (Cosmetic Product): Permissible from 6 years for sensitive skin: no more than 0.5 ml once a day, only as indicated, in the absence of allergic reactions. Under 6 years, not recommended.

Contraindications (Cosmetic Product): Allergy to components, acute inflammatory dermatoses, weeping wounds, individual hypersensitivity to the essential components of the formula. Contraindications during pregnancy and lactation have not been scientifically registered, however, use should preferably be limited to small areas and under control. Not recommended for children under 6 years.

Side Effects (Cosmetic Product): Local redness, burning sensation, itching, peeling, dryness. When the dose is exceeded or applied to damaged skin, increased local inflammatory reaction is possible.

Adjustment for Patient Body Weight (Cosmetic Product): Not required, however, for body weight less than 50 kilograms, it is recommended to reduce the area of application. For body weight more than 90 kilograms, adjustment is not required.

Preparation method (Cosmetic Product): For 100 milliliters: extract of Derris scandens — 2 grams, rosemary hydrolate — 20 milliliters, jojoba oil — 5 milliliters, emulsifier (cetearyl glucoside) — 3 grams, distilled water — 70 milliliters. The components are mixed in two stages: the oil and aqueous phases are heated to 65 degrees Celsius, combined with constant stirring, cooled to 30 degrees. Add the extract, then the hydrolate and preservative (0.5 grams of benzyl alcohol). The finished product is packaged in opaque bottles.

Storage Conditions and Shelf Life (Cosmetic Product): Store at a temperature of 5 to 20 degrees Celsius, in a dark place, away from light and heat sources. Shelf life — 6 months. After opening, use within 30 days, avoiding contact with contaminated surfaces of the dispenser.


Restorative Ointment Form — Derris scandens

Indications (Restorative Ointment): Inflammatory and traumatic skin and soft tissue injuries, subacute dermatitis, local inflammations in joint areas, post-injection infiltrates, bruises, superficial neurodermatitis, skin irritation after physical exertion, scar-adhesive skin changes.

Standard Dosage (Restorative Ointment): Apply 2–3 grams of ointment (a full teaspoon) to the affected area 1–2 times a day. Course — 7–10 days.

Enhanced Dosage (Restorative Ointment): Apply 3–5 grams 3 times a day for pronounced inflammatory skin reactions, protracted post-traumatic infiltrates, and chronic local myositic-dystrophic foci. The maximum period of use of the enhanced regimen is 5 days.

Maximum Dosage (Restorative Ointment): No more than 6 grams per application and no more than 18 grams per day. Use only under dermatological supervision for severe soft tissue inflammations accompanied by edema and pain syndrome.

Preventive Dosage (Restorative Ointment): 1–1.5 grams (half a teaspoon) once a day for 5 days for a tendency to skin inflammations, scarring, to prepare the skin for physical exertion, in patients with professional activity associated with microtraumas. Preventive courses — 2 times a year.

Pediatric Dosage (Restorative Ointment): Permissible from 7 years: 0.5–1 gram of ointment once a day on limited areas, for mild bruises, skin irritation, prevention of chafing. Not used under 7 years.

Contraindications (Restorative Ointment): Acute pustular skin processes, trophic ulcers, open wounds, individual intolerance. Contraindications during pregnancy, lactation, and in children under 7 years have not been scientifically registered, but use should be limited to small doses and short courses.

Side Effects (Restorative Ointment): Rarely: burning, hyperemia, peeling, local pain upon application, allergic reactions when the dosage is exceeded. With prolonged use, contact dermatitis may develop.

Adjustment for Patient Body Weight (Restorative Ointment): Adjustment is not required for external use, but for weight less than 50 kilograms, it is recommended to reduce the area of application and use no more than 1 gram at a time.

With oral intake of extracts, absorption occurs mainly in the small intestine, with partial participation of microflora capable of hydrolyzing glycosidic bonds. Active metabolites undergo phase II metabolism in the liver (conjugation with glucuronic and sulfuric acids), then are distributed in plasma and tissues, mainly in the liver, muscle tissue, and joint structures.

Excretion is carried out primarily through the kidneys with urine in the form of polar metabolites, and to a lesser extent with bile.
