The pharmacodynamic properties of Coleus forskohlii are due to the presence of a unique diterpenoid compound — forskolin, isolated from the root of the plant. Unlike most plant substances, forskolin acts directly on adenylyl cyclase — the enzyme catalyzing the conversion of ATP to cyclic adenosine monophosphate (cAMP), without the participation of receptors on the cell membrane. This distinguishes it from the class of indirect adenylyl cyclase activators and provides specific biochemical activity.

An increase in cAMP concentration activates intracellular signaling cascades, in particular protein kinase A (PKA), which regulates multiple physiological processes in tissues and organs.

Systemically, forskolin exerts an effect at the level of the central and peripheral nervous system, endocrine regulation, smooth muscle tissue, as well as in cells of the immune system. An increase in cAMP in neurons contributes to the modulation of neurotransmitter transmission, including in systems regulated by norepinephrine, dopamine, and serotonin.
