Dosage of Pharmaceutical Forms — Salix alba

Powder — Salix alba

Indications (Powder): Osteoarthritis, rheumatoid arthritis, myalgia, tension headache, dysmenorrhea, fever, hyperthermia, tendinitis, gout, fibromyalgia.

Standard Dosage (Powder): 1 gram of powder 2 times a day, with warm water, 30 minutes before meals. Duration of intake — up to 14 days.

Enhanced Dosage (Powder): 1.5 grams of powder 3 times a day for 10 days for acute inflammatory joint diseases, fever, and pronounced myalgia.

Maximum Dosage (Powder): Do not exceed 6 grams of powder per day. Allowed for no more than 3 days for fever, infectious diseases of the upper respiratory tract, and pronounced pain syndrome.

Preventive Dosage (Powder): 0.5 grams of powder once a day in the morning on an empty stomach. Recommended for chronic osteoarthritis, recurrent bursitis, a tendency to gout, patients over 60 years of age — in courses of 10 days once a month.

Toxicity and Biosafety of Salix alba

White willow bark (Salix alba) demonstrates a high biosafety profile upon oral and external use in therapeutic doses. The main active substance is considered to be salicin — a natural precursor of salicylic acid. Unlike synthetic salicylates, salicin in willow bark is metabolized more slowly, which reduces the risk of acute toxic effects.

In a toxicological assessment study of white willow extract on mice and rats, conducted in accordance with OECD Guidelines, it was established that the acute oral toxicity LD₅₀ value of Salix alba bark extract is more than 5000 mg/kg body weight in rats, which indicates extremely low toxicity of the substance.

With external use on the skin, no signs of irritation, photosensitization, or cumulative toxicity have been detected. There are isolated reports of possible allergic reactions in individuals with intolerance to salicylates.

White willow extracts do not exhibit mutagenicity, cytotoxicity, or embryotoxicity in standard in vitro and in vivo test systems. Modern pharmacological reviews classify Salix alba as a non-toxic herbal preparation with a favorable safety index.

Pharmacodynamics — Salix alba

The pharmacodynamic properties of white willow are due to a complex of phenolic compounds, among which the main active component is considered to be salicin. Pharmacokinetics — Salix alba

The pharmacokinetics of the active substances of white willow is determined by the physicochemical properties of salicin and associated polyphenolic components. Upon oral administration, salicin in unchanged form passes through the stomach and is absorbed in the small intestine, where it undergoes hydrolysis by microflora with the formation of saligenin. The latter is oxidized in the liver to salicylic acid — the main biologically active metabolite. The entry of active substances into the systemic circulation occurs primarily through the portal vein, with the first metabolic passage through the liver. The distribution of metabolites occurs evenly, with accumulation in tissues with a high degree of inflammation and oxidative stress. Due to low lipophilicity, the main metabolite — salicylic acid — penetrates poorly through the blood-brain barrier but exhibits activity in peripheral tissues.

These compounds may partially undergo enterohepatic circulation, which increases the duration of their action.

Individual studies indicate antiproliferative and modulating properties of white willow, due to the interaction of flavonoids with the aryl hydrocarbon receptor (AhR), which regulates cellular detoxification and immune homeostasis. An effect on the expression of the enzyme indoleamine-2,3-dioxygenase (IDO), involved in the immunometabolism of tryptophan and regulation of tolerance, has also been demonstrated.

At the cellular level, the ability of the active substances of willow bark to reduce the migration of neutrophils and macrophages in the focus of inflammation has been confirmed, as well as to suppress the expression of the enzyme matrix metalloproteinase-9 (MMP-9), involved in the remodeling of the extracellular matrix and chronicity of inflammation.

Thus, the pharmacological mechanisms of action of white willow include suppression of enzymatic pathways of inflammation (COX, LOX), blockade of mediator transmission (prostaglandins, cytokines), antioxidant stabilization of cellular structures, as well as immunotropic regulation through NF-κB and AhR signaling cascades. These effects are realized mainly at the level of macrophages, neutrophils, endothelial cells, and receptor targets of the inflammatory profile.

Synergy — Salix alba

Pharmacological synergy of white willow with other plant and natural compounds has been confirmed by numerous preclinical and laboratory studies. One of the most studied areas is the potentiation of anti-inflammatory action when used together with polyphenolic compounds of the flavonoid series. Combined use of white willow and curcumin has shown an increase in the suppression of COX-2 expression and prostaglandins, as well as a more pronounced inhibition of NF-κB signaling in macrophages, which confirms the additive and potentiating nature of the interaction.

Synergy with boswellia extracts (Boswellia serrata) has also been identified, in which a more sustained reduction in the production of leukotrienes and thromboxane B2 is achieved due to double inhibition of LOX and COX cascades. This combination has demonstrated tissue-specific enhancement of the effect in connective tissue structures and in the vascular endothelium.

In experimental models, potentiation of sedative and anxiolytic action has been established when white willow is combined with extracts of lemon balm and passionflower. This effect is due to the action of flavonoids on gamma-aminobutyric acid (GABA) receptors and suppression of adenylate cyclase activity, which enhances inhibitory neuromodulation.

Thus, white willow can enter into potentiating, additive, and modulating interactions with a number of natural substances. Geography of Use and Traditional Medicine — Salix alba

Salix alba has a long history of use in the folk medicine of Europe, Asia, and the Middle East. One of the first regions where the plant gained recognition as a medicinal remedy was the territory of Ancient Greece: references to the use of willow bark are contained in the works of Hippocrates (5th century BC), where properties similar to modern ideas about plants containing salicylates are described. In the medieval European tradition, willow bark was actively used in monastic medicine — especially in Benedictine scriptoria in France and Germany, where it was part of decoctions used to maintain strength and reduce fever. In the manuscripts of the Salerno school, recommendations for its external use in the form of wraps and compresses have also been preserved.

The Slavic peoples, especially in the territory of Ancient Rus', used willow branches and bark for the preparation of infusions, applications, and bath decoctions. In Russian village tradition, willow was considered a "women's tree," associated with purification and restoration; its branches were used in ritual baths on the days of the spring equinox. Ethnography records the use of willow bark decoction for fumigating premises in order to "remove the evil eye" and neutralize negative energy.

In Islamic traditional medicine, willow was known as "shajarat as-safsaf" and was used in the Maghreb and Persia as a remedy capable of "cooling the blood" and soothing "internal heat." Bark decoctions were part of multicomponent recipes in the school of Avicenna, in particular in combination with mint and pomegranate.

In Tibet and Mongolia, willow bark was used as part of powders and oils used for rubbing into the joints, and also as a component of medicinal fumigations. In the ritual practice of Bon and Lamaism, willow branches were considered a tree of purification, used in ritual purifications from "spirits of heat" and for tying altars during spring festivals.

In Chinese ethnomedicine, the species Salix alba is less common; however, in the northern provinces (Shandong, Hebei), it was part of the recipes of local healers, mainly as a component of anti-dampness infusions and water baths. In Nepal and northern India, trees of the genus Salix were used to create pastes based on ground bark, which were applied to the body after physical exhaustion.

In Thailand and Laos, according to QSBG, willow trees are considered a secondary source for folk medicine, but branches and bark may be used for rubs, especially in mountainous areas with a Chinese population. In the culture of the Carpathian Hutsuls and Moldovans, willow was used in spring-summer rituals: wreaths were woven from its branches, which were burned on Kupala to cleanse the home of diseases.

Thus, white willow acts as one of the oldest ethnomedical resources of Eurasia, the use of which covers a wide range of regions, cultures, and ritual practices. Its forms of application vary from decoctions and pastes to wraps, fumigations, and ritual fumigation.
