Standard Dosage (Powder): 1000 mg (1 g) 2 times a day orally after meals, with warm water. Course from 14 to 28 days.

Enhanced Dosage (Powder): 2000 mg (2 g) 2 times a day for myositis, periarthritis, tendinitis, and chronic osteoarthritis in the active phase. Course up to 21 days.

Toxicity and Biosafety of Zingiber cassumunar

Experimental in vivo data demonstrate low overall toxicity of Zingiber cassumunar upon oral and external use.

The acute toxicity (LD₅₀) of aqueous and alcoholic extract of Zingiber cassumunar rhizome in laboratory rats upon oral administration exceeds 5000 mg/kg body weight, which is classified as a practically non-toxic substance.

External application of essential oil and oil infusion in therapeutic doses did not cause skin irritation or sensitization in tests on guinea pigs and rabbits. Repeated administration of the extract at a dose of 1000 mg/kg for 28 days did not show histological changes in the liver, kidneys, spleen, and other organs of experimental animals.

Thus, Zingiber cassumunar has a favorable toxicological profile and can be considered safe when the recommended dosages are observed.

Pharmacodynamics — Zingiber cassumunar

Cassumunar ginger exhibits multicomponent pharmacodynamic action, mainly of local and systemic level. The main bioactive compounds of the taxon — sabinene, terpinenes, cassumunarin, and β-gurjunene — participate in the modulation of inflammatory mediators and enzymatic activity. The pronounced anti-inflammatory effect is explained by inhibition of cyclooxygenase (COX-2) and lipoxygenase (5-LOX) enzymes, leading to a decrease in the production of prostaglandins and leukotrienes. This affects the activation of tissue macrophages, neutrophils, and a decrease in capillary permeability.

Immunotropic properties are realized through suppression of cytokine production, including TNF-α and IL-6, which contributes to a decrease in the expression of NF-κB inflammatory signaling pathways. These effects form the basis of the anti-edema and reparative action with local use. In dermal and transdermal forms, Zingiber cassumunar stimulates microcirculation, exerts a muscle-relaxing effect, and reduces the release of neuropeptides associated with pain impulses in the skin and muscles.

The pharmacokinetic behavior of the active substances of Zingiber cassumunar is determined mainly by the form of administration and the polarity of the constituent components. Mechanisms of Action and Scientific Rationale — Zingiber cassumunar

The pharmacological activity of Zingiber cassumunar is due to the combined action of its bioactive compounds, including sabinene, cassumunarin, β-gurjunene, terpinenes, and curcuminoid derivatives. The key direction of action is the inhibition of pro-inflammatory mediators through inhibition of the enzymatic activity of cyclooxygenase-2 (COX-2) and 5-lipoxygenase (5-LOX), which reduces the synthesis of prostaglandins and leukotrienes. This affects the level of inflammation, reducing the activation of macrophages, neutrophils, and the expression of adhesion molecules on the endothelium.

Additionally, Zingiber cassumunar compounds suppress the nuclear transcription factor NF-κB, blocking the transcription of pro-inflammatory cytokine genes (TNF-α, IL-1β, IL-6), and also modulate the MAPK/ERK pathway, reducing the phosphorylation of p38 and JNK. These cascades regulate the production of nitric oxide (NO) and the expression of iNOS, which provides anti-edema and analgesic action. In the extract, an effect on TRPV1 receptors of sensory neurons has been noted, which is accompanied by a decrease in peripheral nociception. Thus, modulation of these receptors exerts a neurotropic effect, reducing the sensitivity of pain endings and decreasing the local neuroinflammatory response.

The antioxidant mechanism of action is associated with the direct radical-scavenging effect of curcuminoid compounds, activation of superoxide dismutase (SOD) and glutathione peroxidase, as well as suppression of lipid peroxidation. In vitro studies confirm that Zingiber cassumunar components can bind to iron-containing enzymes and chelate metal ions, reducing oxidative stress at the cellular level.

An effect on the immune system has been proven, in particular, suppression of T-lymphocyte proliferation and a decrease in HLA-DR expression on activated macrophages, which indicates a mild immunomodulatory effect. A decrease in the activity of β-glucuronidase and elastase, involved in the destruction of the extracellular matrix and the spread of inflammation in tissues, is also observed. In dermal and transdermal forms, Zingiber cassumunar oils and extracts contribute to an increase in local blood flow, a decrease in muscle spasm, and stimulation of regenerative processes by improving the delivery of oxygen and nutrients to the tissue. Thus, the pharmacological action of Zingiber cassumunar is realized through multiple targets: COX/LOX enzyme systems, NF-κB signaling pathways, MAPK, TRPV1 receptors, inflammation mediators, and antioxidant defense, confirmed by biochemical and cellular models.

Synergy — Zingiber cassumunar

Pharmacological synergy of Zingiber cassumunar is manifested in combination with other medicinal plants and substances with anti-inflammatory, analgesic, and antiseptic action. It has been proven that combined use with Curcuma longa (turmeric) leads to potentiation of COX-2 inhibition and suppression of NF-κB expression, which significantly enhances the anti-inflammatory and anti-edema effect in a tissue inflammation model. This effect has been confirmed both in vitro and in vivo.

Synergy with Andrographis paniculata is realized through additive suppression of pro-inflammatory cytokines and increased expression of antioxidant enzymes. This interaction enhances systemic antioxidant protection and modulates JAK/STAT signaling cascades. Simultaneous effect on the cytokine profile reduces IL-6, IL-1β, and TNF-α levels more effectively than isolated use of each plant.

In combination with essential oils of Cinnamomum camphora and Eucalyptus globulus, an enhancement of the local hyperemic and analgesic action is observed. Such mixtures are used in balms and patches, and demonstrate a potentiating effect due to accelerated penetration through the skin and increased bioactivity of volatile components. In vitro studies confirm an increase in skin membrane permeability and enhanced inhibition of bacteria of the genus Staphylococcus and Candida when combined with Melaleuca alternifolia (tea tree) oils.

A modulating interaction with curcuminoids and terpenoid complexes has also been noted, due to which the synthesis of endogenous antioxidants, including glutathione, is activated, and the cell membrane is stabilized under conditions of oxidative stress. Thus, Zingiber cassumunar demonstrates pronounced pharmacological synergy with anti-inflammatory, antioxidant, and antiseptic components of both plant and mineral origin, which makes it a promising component of multicomponent preparations and phytocomplexes.

Geography of Use and Traditional Medicine — Zingiber cassumunar

Cassumunar ginger has traditionally been used in the territory of Southeast Asia, mainly in Thailand, Cambodia, Laos, Myanmar, and Indonesia. In Thai traditional medicine, it is known as "plai" (ไพล) and is considered one of the basic plants in formulations for external use for physical exertion and recovery after injuries. Its use dates back to medical treatises of the Ayutthaya era (14th–18th centuries), where it was mentioned as a remedy for rubs and compresses as part of monastic oil formulas.

In the folk medicine of northern Thailand and Isan, fresh or dried plai rhizomes were crushed and infused in coconut or rice oil, used in the form of compresses, rubs, or wraps. Especially often they were used in postpartum care: women underwent a course of external warming using hot compresses, which included cassumunar ginger. It was also used for inhalations — the evaporation of rhizomes with other herbs was considered cleansing for spirit and body, used for the treatment of fatigue and during rituals of exorcising evil spirits.

Among the Karen and Lanna peoples living in the northern mountainous regions of Thailand and Myanmar, cassumunar ginger was considered a plant of strength and was part of protective ointments applied by hunters and warriors before entering the forest. It was used also as a component of ritual house cleansing: dried slices of rhizomes were burned in the hearth as part of a ritual of exorcising illness, often together with turmeric roots and lemongrass stems.

In the Cambodian tradition, the plant was used as a universal remedy in steam baths: whole rhizomes were placed in vessels with boiling water, through which steam passed, inhaled by the patient or directed onto the body. In Indonesian therapeutic massage (urat tradisional), known in Java and Bali, a warm paste-like composition of plai and other roots was applied to the skin and wrapped with cloth.

Historically, the use of Zingiber cassumunar is recorded in medical texts on palm leaves found in northeastern Thailand, as well as in temple murals depicting herbalists mixing ingredients similar in appearance to plai rhizome. In addition to medicinal use, the plant also had ritual significance — its smell was considered cleansing from evil influences, and in some areas the roots were placed under the pillow of a sick person to protect sleep.
